Education: High School Diploma or GED equivalent required. Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Nursing is preferred.
Description: This position will provide clinical and administrative supervision to our new team of Health Promotion Nursing staff and Health Promotion Coordinators. As this is a new program, will work collaboratively with the Director of Population Management to identify referral process. Will provide expertise and support for integrated behavioral health and primary care, and substance use. This position will identify, develop and implement nursing processes and service delivery goals. Responsible for identifying through a systematic process, patients who benefit from health promotion care coordination by using criteria that consider (1) behavioral health conditions, (2) chronic medical conditions, (3) certain social determinants of health and (4) high use/ high costs of healthcare services. Populations serviced by care management have a high prevalence of behavioral health conditions/issues. This position will also serve a critical collaborative function with community partners for primary and specialty care services, the ongoing development of integrated care, and programming to further health and wellness for clients across the agency. Will also serve as a point of review and monitoring of clinical nursing incidents under supervision of the Director of Nursing and in coordination with the medical leadership team. At this level, work is performed with extensive latitude for the use of initiative and independent judgment. Flexible schedule may be required to meet the demands of the program.
The ideal candidate will have at least one year of medical surgical nursing and three to five years' experience providing care to patients with mental illness and substance use disorders and or individuals with IDD. At least a year of supervisory/management and/or quality improvement experience is preferred.
License(s): Required to be licensed as a Registered Nurse in the State of Texas. Current and valid Texas driver's license and a clean driving record required.
Hours: Monday-Friday, 8:00am-5:00pm.
Salary: $83,298 annually.
