Community Health Worker (QMHP)

Job Summary: The Community Health Worker (CHW) will represent Integral Care in the community providing outreach and education about the Community Health Worker role to stakeholders. CHW will promote communication and understanding between community members and providers, increase the use of health services in a community, improve adherence to health recommendations, and even decrease emergency services. The CHW will help families in the community understand and recognize early signs of mental illness and substance use disorder, encourage community members to get appropriate screenings for behavioral health conditions and hold group workshops on mental wellbeing.

Community Health Worker (CHW) position will be located in the community and in settings where individuals receive services and live. Interventions and services may be in the community or the office and may require the use of a personal vehicle. CHW will assist individuals with navigation and accessing resources that promote a healthy lifestyle per the Social Determinants of Health. CHW will work with individuals in developing skills necessary to manage symptoms, obtain/maintain services such as medical care, nutritional assistance, financial assistance, transportation, legal assistance, housing, or other resources. Individuals served at this level are assessed for crisis services as needed. CHW will promote, maintain, and improve the health of individuals by providing informal counseling and education specific to the individual's needs. CHW will be responsible for providing recovery-focused, person-centered care with a trauma-informed perspective. For individuals enrolled in care, the CHW will work closely with the treatment teams to promote care continuity and address gaps in overdue labs and screenings, hospital follow-up care, and access to other services. CHW will be working under an LPHA that provides ongoing evaluation and oversight of all assessments, diagnoses, and treatment.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.

License:

  • Current and valid driver's license and a clean driving record.

Hours: Full-Time.

Workdays: Monday-Friday.

Workhours: 8:00AM - 5:00PM.

Salary: $22.35 - $23.47an hour based on education.
