Job Summary: Within Integral Care's Intellectual and Developmental Disability (IDD) Division, the HUB DADS Transition Support Team (TST) program is seeking a licensed professional as the program lead in order to be in compliance with contract requirements per program standards from HHS and CMS. The program oversees a region of 27 counties, including 5 Local IDD Authorities and the IDD private providers for HCS and TxHml programs across this region.
The TST program contract, per HHS and CMS standards, requires a licensed professional as the full-time lead for the program. The TST program fulfills contracted services through the federal Money Follows the Person grant for a region of 27 counties, including 5 Local IDD Authorities and the HCS and TxHml private providers across this service area. This LPHA Supervisor position involves direct and detailed oversight of this program as necessary to fulfill contract requirements.
Program services include clinical case reviews and consultations, which require clinical supervision, as well as leadership of an interdisciplinary team of clinical professionals, including but not limited to a psychiatrist, psychologist, LCSW, BCBA, and Occupational Therapist. The supervisor position for the TST Program directly supervises an FTE LPHA position, which requires LPHA clinical supervision. This position supports correspondence with HHSC for the TST on behalf of the designated area, including proposing and seeking approval for TST projects; providing programmatic updates and submitting quarterly reports; providing planned budgets; and providing information regarding any budgetary changes.
Licensed Professional of the Healing Arts (LPHA) embraces trauma-sensitive and -informed practices, provides Suicide Safe care, and fosters a supportive and welcoming environment. LPHAs operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill set by forging therapeutic relationships with community members and/or stakeholders. Interventions and services can be both office- and community-based and may require the use of a personal automobile. Individuals served may be diagnosed with mental illness, co-occurring substance use disorders, chronic health conditions, and/or IDD.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Salary: $81,912.81 - $104,430.64, based on licensure.
