Case Manager - QMHP

Job Summary: This role provides services to individuals diagnosed with severe mental illness, many of whom may also experience co-occurring substance use disorders and/or chronic health conditions. Services are delivered using a recovery-oriented and trauma-informed approach. The position operates in a dynamic environment that requires extensive knowledge of community resources, collaboration with interdisciplinary teams, and the ability to build therapeutic relationships with diverse community members. Services may be provided in both office and community settings and may require the use of a personal automobile and cell phone. 

The position partners with racially and culturally diverse individuals to identify and support their desired life outcomes. Responsibilities include participating in diagnostic evaluations and assessing strengths, preferences, functional impairments, and symptoms related to psychiatric and/or co-occurring disorders at regular intervals to ensure personalized and effective care. Key responsibilities also include assisting individuals in developing skills to manage symptoms, maintain employment or housing, and access essential services such as medical care, nutritional assistance, financial support, transportation, and legal resources. Depending on the assigned unit, the role may also involve facilitating psychosocial rehabilitation skills groups and implementing evidence-based interventions or therapeutic modalities. Individuals served may require crisis service assessments as needed. While this role is not typically designated as a Licensed Practitioner of the Healing Arts (LPHA), all services are provided under the supervision of an LPHA who offers ongoing evaluation and oversight of assessments, diagnoses, and treatment. 

Additional duties may include providing bilingual communication services. This requires successful completion of a language competency assessment

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. Master's degree preferred.

License:

Required:

  • Current and valid driver's license and a clean driving record.

Hours: Full-Time.

Workdays: Monday - Thursday or Monday - Friday.

Workhours: 7:00AM - 6:00PM

Salary: $24.18- $25.39 an hour based on education.
