Job Summary

Case Managers provide services to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions, utilizing a recovery-based and trauma-informed perspective. In this role, they operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate clinical skills by forging therapeutic relationships with diverse community members.

Services may be delivered in both office and community settings and can require the use of a personal automobile and cell phone. Case Managers partner with racially and culturally diverse individuals to define and facilitate their desired life outcomes.

Responsibilities include participating in diagnostic evaluations and conducting ongoing assessments of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorders to ensure optimal, individualized services. They support individuals in developing skills necessary to manage symptoms; obtain and maintain employment or housing; and access resources such as medical care, nutritional assistance, financial assistance, transportation, legal services, and other basic needs.

Depending on unit assignment, the role may also include facilitating psychosocial rehabilitative skills training groups and/or implementing other evidence-based interventions or therapeutic modalities. Individuals served at this level may be assessed for crisis services as needed.

Although this position is not typically designated as a Licensed Practitioner of the Healing Arts (LPHA), services are provided under the supervision of an LPHA who offers ongoing evaluation and oversight of assessments, diagnoses, and treatment. Additional responsibilities may include providing bilingual communication services, which requires successful completion of a language competency test.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field required. Master's Degree preferred.

License:

Required:

Current and valid driver's license and a clean driving record.

Hours: Full-Time.

Workdays: Monday - Friday

Workhours: 8:00AM - 5:00PM.

Salary: $24.18 - $25.39 an hour, based on education.
