Rehabilitation Specialist (QMHP)

Job Summary: This position is within Housing and Homeless services as part of the Mobile Medical and Mental health (M3) team. This project works in partnership with Community Care Health Center and Dell Medical School to fill service gaps for adults experiencing chronic homelessness with a chronic medical condition and co-occurring disorder of serious mental illness and substance use disorder. Individuals served present with high vulnerability and complex needs. Work will be conducted within an interdisciplinary team to address needs/skills around housing navigation and stability, and related service coordination including delivering crisis services. The Rehabilitation Specialist (QMHP) will utilize the evidence-based practices of Housing First, Integrated Dual Disorder Treatment, Trauma Informed Care and Harm Reduction. This position will work in partnership with racially and culturally diverse clients to provide flexible interventions aligned with individual stage of change. The Rehabilitation Specialist (QMHP) will build on strength by fostering an attitude of hope, respect and compassion. The M3 team will work collaboratively with Landlord Outreach Specialists, Housing Authorities, Assertive Outreach teams, Supported Employment, and other local service organizations. The M3 team is mobile meaning services are provided in the field, including outreach sites, within Travis County. This position requires the transportation of consumers in personal vehicle.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. 

Preferred:

  • Graduated from an accredited college or university with a Master's degree with a major specialization in Human/Behavioral/Social Services or related field. 

 License: 

Required:

  • Current and valid driver's license and a clean driving record. 

Hours: Full-time; 

Workdays: Monday-Friday. 

Workhours: 8:30 AM - 4:30 PM.

Salary: $22.35 - $23.47 an hour based on education.
