Job Summary: The Positive Assistive Community Engagement (PACE) Program serves people diagnosed with intellectual and/or developmental disabilities who live in a variety of non-institutional settings who are experiencing a crisis. Services are designed to meet the person's needs so they may remain in the community and have opportunities to participate in their community to the maximum extent possible.

As a PACE staff member you will provide services and supports for individuals diagnosed with intellectual and/or developmental disabilities (IDD). In addition to IDD, many individuals you will serve will have mental health and/or medical diagnoses. Individuals served range in age from very young children to the geriatric population.  Services will only be provided in what is currently considered the individual's home setting (group home, family home, own home, or host home). The position is a dynamic position that entails working with the individuals and their support systems, focusing on crisis prevention and advocacy. This position will also include supporting other IDD crisis prevention teams on as needed basis.

Responsibilities and Duties:

• Identify and leverage clients own strengths to help them achieve their goals with a focus on crisis prevention
• Work with clients experiencing a current crisis that may have the potential for out of home placement in a higher level of care facility (hospital, state facility, correctional facility).
• Work 1:1 with individuals and coordinate therapeutic interventions focused on de-escalation, safety planning, emotional regulation, and coping skills. 
• Maintain documentation of all client contacts within required timelines and in compliance with program requirements
• Develop working relationships with other Integral Care staff to ensure appropriate client services and to share effect therapeutic interventions. 
• Assess for safety and complete assessments 
• Provide support to individuals in accessing appropriate services through communication within Integral Care, community partners, service providers and other relevant agencies.
• Travel locally, daily, for client visits
• Participate in inter-agency collaborations to problem solve client needs and barriers to progress
• Utilize electronic health records, Microsoft Word/Outlook and other applications.

Qualified Mental Health Professional (QMHP) Education:

Required:

• Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. 

Qualified Mental Health Professional (QMHP) License: 

Required:

• Current and valid driver's license and a clean driving record. 

Hours: Full-time; Monday-Friday, 10:00am-7:00pm 

Salary:  $21.96 - $22.99 an hour based on education. "Internal applicants are subject to Integral Care's QMHP career ladder".

This position is eligible for differential pay.

• Second Shift: $3.25

• Third Shift: $4.25

• Weekend Shift: $5.00
