Education and Special Projects Specialist Job Summary: The Education and Special Project Specialist will facilitate services for the Transition Support Team (TST), a program through Money Follows the Person, a federal grant. The Transition Support Team at Integral Care serves professionals working with individuals with IDD over a designated service area (DSA) of 27 counties. Services include educational opportunities, case consultation, and technical assistance. TST services aim to promote quality care and supports for individuals with an intellectual and/or developmental disability who are at risk for institutionalization or are transitioning into the community from an institutional setting. The Education and Special Project Specialist will work closely with the TST Team Lead to implement the program as described above.
Additionally, the Education and Special Project Specialist will work alongside the TST Team Lead to increase the capacity of IDD and health professionals in serving the unique needs of individuals with IDD diagnoses through education, consultation, and technical assistance. As the Education and Special Projects Specialist, you will provide services in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams and cultivate a unique skill-set learning from, coordinating with, and forging relationships with diverse community partners. You will provide services to professionals across 27 counties across the state of Texas.

Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479 - $40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
