Continuity of Care Navigator - QMHP

Job Summary: 

Continuity of Care Navigators perform moderately complex coordination work, provide preliminary assessment of a client's presenting psychiatric and/or co-occurring disorder(s), develop a plan to provide linkage to services sought or needed by the client, and to coordinate appropriate services to facilitate each client's success in achieving and maintaining the maximum possible level of independent functioning. This position is community based and provides short term services to adult clients to provide linkage and advocacy in establishing care with Integral Care and partners such as Community Care and Central Health. The services provided will focus on appropriately linking clients across a continuum of care, which primarily includes Community Care, Integral Care, and Central Health. This role has a primary focus on advocating to ensure client connection to ongoing care for any indicated needs.

This position serves individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective. The services that Continuity of Care Navigators provide require frequent coordination with external and internal providers as necessary to support individuals in achieving independent functioning. Continuity of Care Navigators provide coordination to assure that the individual is connected to any indicated services. Services may include coordination to assist the client in obtaining or maintaining employment, housing, medical care, dental care, food, clothing, utilities, financial assistance, transportation, legal assistance, or any other basic human need.

Although staff members in this position are not typically LPHAs, staff members in this position are working under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, crisis intervention, caseloads and client coordination. This position may have additional duties associated with providing bilingual communication services and requires the completion of a language competency test. Interventions and services are typically provided in the community and require the use of a personal automobile and cell phone.

Regular, predictable attendance is considered to be an essential requirement for this position.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. Master's degree preferred.

License:

Required:

  • Current and valid driver's license and a clean driving record.

Experience:

Preferred:

  • Bilingual preferred.

Hours: Full-Time.

Workdays: Monday - Friday.

Workhours: 8:00 AM - 5:00 PM 

    Salary: $24.18 - $25.39 an hour based on education.
