IDD Crisis Intake and Enrollment Coordinator

Job Summary: The IDD Crisis Intake and Enrollment Coordinator (IDD Crisis I&E Coordinator) will support individuals residing in Travis County seeking IDD eligibility determination and/or enrollment into the IDD waiver program Home and Community Based Services (HCS). 

As an IDD I&E Coordinator, you will provide services in the community, including individual's place of residence, schools, day programming, as well as within the crisis respites, psychiatric hospitals, jails, and homeless shelters/camps.  Many of the individuals you will serve will have severe mental illness diagnoses, substance use disorders, chronic health conditions and/or experience chronic homelessness.  All individuals will have experienced, are currently experiencing, or at risk of experiencing crises including psychiatric hospitalization, incarceration, or institutionalization at a State Supported Living Center or Nursing Facility. 

By managing a fixed caseload of individuals and providing robust and intensive support, the IDD Crisis I&E Coordinator will work to decrease the time it takes to determine eligibility for ongoing IDD services and/or enroll in the HCS waiver program.  And in doing so, the IDD Crisis I&E Coordinator will support individuals with IDD remain safely in the community and reduce the risk of future crises. 

Responsibilities and Duties:

  • Assist individuals completing an initial IDD intake in gathering the required clinical, financial, and identification documents to support Determination of Intellectual Disability (DID) assessments, such as copies of previous psychological evaluations; any evaluations from school (e.g. Full & Individual Evaluation and ARD meeting paperwork); neurological or genetic evaluations (e.g. an evaluation from 'Specially for Children); Comprehensive Diagnosis & Evaluation (D&E); any documentation referring to a diagnosis of intellectual disability or autism/pervasive developmental disorder (PDD). 
  • Assist individuals in completing all intake and enrollment related appointments by working to eliminate scheduling and transportation related barriers.
  • Assist individuals in scheduling and completing the Determination of Intellectual Disability (DID) eligibility testing appointment including providing appointment reminders, collaborating with caregivers, and providing transportation support. 
  • Assist individuals at risk of institutionalization complete an HCS diversion application based on medical or behavioral health need.
  • Assist individuals enrolling into the HCS waiver program schedule an initial or updated DID appointment; apply for Medicaid if needed; provide support in identifying and choosing an HCS Provider; complete Person-Directed Plan and Individualized Plan of Care needed to initiate HCS services.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.

License:

Required:

  • Current and valid driver's license and a clean driving record.
  • Center certification to perform service coordination. (Training will be provided during the onboarding process.)

Experience:

Preferred:

  • Bilingual in Spanish.

Hours: Full-time; Monday-Friday, 8:00am-5:00pm with some flexibility.

Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.

Position is eligible for differential pay.

  • Second Shift: $3.25
  • Third Shift: $4.25
  • Weekend Shift: $5.00
