Village and provides mental and behavioral health services, substance use support, and case management to its residents. 

The Housing Stability Specialist assists vulnerable individuals who have experienced chronic homelessness, helps them transition to housing successfully, fosters independence, and cultivates important living skills.  The people we work with are racially and culturally diverse and often have complex needs, including psychiatric and/or co-occurring substance use disorders, and chronic medical conditions.  Housing Stability Specialists provide psychosocial rehabilitative skills training, emotional support, intensive case management, and service coordination, utilizing a person-centered, trauma-informed, recovery focused approach to help these individuals define and facilitate their desired life outcomes.

Housing Stability Specialists participate in diagnostic evaluations at regular intervals and provide assessment of individual strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring substance use disorders to ensure optimal and personalized services.  Frequent service coordination with external and internal providers is necessary to support individuals in achieving independence and may include access to medical care, substance use treatment, nutrition, financial assistance, transportation, legal assistance, or other services as required to meet their goals and/or basic needs.  Housing Stability Specialists may also facilitate psychosocial rehabilitative skills training groups to help individuals develop more effective coping strategies and achieve their recovery goals.  This position carries a high intensity caseload in addition to serving community residents on an outreach and on-call basis when needed.   

Housing Stability Specialists work under general supervision with moderate latitude for the use of initiative and independent judgment.  Although staff members in this position are not typically LPHA's, staff members in this position work under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses, and treatment.  This position requires the transportation of consumers in a personal and/or agency vehicle.  

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. 

Preferred: 

  • Graduated from an accredited college or university with a Master's degree with a major specialization in Human/Behavioral/Social Services or related field 

License:

Required:

  • Current and valid driver's license and a clean driving record.
  • Center certification to perform service coordination.
Preferred: 
  • LPC-Associate or LMSW License.

Experience:

Preferred:

  • Two years community mental health experience.
  • Bilingual in Spanish

Hours: Full-time; 8:00 AM - 5:00 PM with flexibility. 

SalaryStarting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
