Service Coordinator (QMHP)

Job SummaryThis position is located in any direct service division of Integral Care.  The Service Coordinator performs moderately complex case management/service coordination work assisting clients in accessing/linkage to medical, social, educational, financial, and other appropriate services/supports that may help individuals achieve a quality of life and community participation acceptable to the individual/legally authorized representative (LAR).  This position works under general supervision with moderate latitude for the use of initiative and independent judgment.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.

License:

Required:

  • Current and valid driver's license and a clean driving record.
  • Center certification as a Qualified Mental Health Profession (QHMP). (This certification will be provided by Integral Care if educational requirements are met.)

Hours: Full-time. 

Workdays; Monday-Friday. 

Workhours; 8:00am-5:00pm.

Salary: $22.35 - $23.47 an hour based on education.

Work may require light to moderate carrying (10 to 45 pounds), simple grasping, dual simultaneous grasping, sitting, standing, twisting, kneeling, stooping, walking, climbing stairs/ladders and reaching above shoulders. Capable of working closely with others or alone. Ability to see, read, write, count, tell time and operate office equipment. May require travel.

Assists individuals with Medicaid applications and notification of potential Medicaid lapses

Ability to develop productive relationships with individuals, family members and service planning team members that focus on consumer specific outcomes. 

In order to meet compliance with Medicaid Administrative Claiming policies, is involved in program planning, policy development and interagency coordination of Medicaid and non-Medicaid service programs. Thus, the position may participate in Time Studies as appropriate.
