Job Summary: This position is located in any direct service division of Integral Care. The Service Coordinator performs moderately complex case management/service coordination work assisting clients in accessing/linkage to medical, social, educational, financial, and other appropriate services/supports that may help individuals achieve a quality of life and community participation acceptable to the individual/legally authorized representative (LAR). This position works under general supervision with moderate latitude for the use of initiative and independent judgment.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Salary: $22.35 - $23.47 an hour based on education.

Assists individuals with Medicaid applications and notification of potential Medicaid lapses
Ability to develop productive relationships with individuals, family members and service planning team members that focus on consumer specific outcomes.
In order to meet compliance with Medicaid Administrative Claiming policies, is involved in program planning, policy development and interagency coordination of Medicaid and non-Medicaid service programs. Thus, the position may participate in Time Studies as appropriate.
