Job Summary:

The TxHmL program provides supports so individuals with intellectual or developmental disabilities can live in their own or family homes. Nursing services are one of the reimbursable supports under this program.

The Registered Nurse (RN) provides nursing services and clinical oversight for individuals enrolled in the Texas Home Living (TxHmL) waiver program. The RN is responsible for assessing health needs, developing and monitoring nursing plans of care, providing direct nursing services as authorized, delegating nursing tasks, and ensuring compliance with Texas Health and Human Services Commission (HHSC) requirements and Texas Board of Nursing standards. 

Essential Duties and Responsibilities include: 

  • Conduct comprehensive nursing assessments and ongoing evaluations of individuals served in the TxHmL program. 
  • Develop, implement, and update individualized nursing plans of care based on assessed needs and physician orders. Provide direct nursing services as authorized in the individual's service plan.
  • Monitor individuals for changes in medical or health status and coordinate appropriate interventions. 
  • Administer medications and treatments or delegate tasks in accordance with Texas Board of Nursing delegation rules. 
  • Provide health education to individuals, families, and direct care staff, including medication management and health-related supports. 
  • Coordinate care with physicians, pharmacists, case managers, service coordinators, and interdisciplinary team members. Participate in service planning team meetings and contribute nursing recommendations. 
  • Maintain accurate, timely, and compliant documentation in accordance with HHSC TxHmL and billing requirements. 
  • Ensure nursing services meet quality assurance standards and regulatory compliance. 
  • Respond to medical concerns, incidents, or emergencies as required. 

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Nursing.

License:

Required:

  • Current and valid driver's license and a clean driving record.
  • Licensed as a RN in the state of Texas.

Experience:

Preferred:

  • One- three years' experience providing direct care for active substance abusers, mentally ill or developmentally or intellectually disabled individuals.

    Hours: As needed.

    Workdays: Monday - Friday.

    Workhours: As needed.

    Salary: $37.87 - $50.69 an hour depending on years of licensure.

    Position may be eligible for an additional shift differential pay. 

    • Second Shift: $3.25/hour. 
    • Third Shift: $7.25/hour. 
    • Weekend Shift: $8.00/hour.
