Service Coordinator Job SummaryThe Texas Home Living (TxHmL), General Revenue (GR), and Community First Choice (CFC) programs provide essential services and supports so that people with IDD can continue to live with their families or in their own homes.  The programs serve individuals diagnosed with Intellectual and Developmental Disabilities in Travis County. 

Integral Care provides service coordination for the programs listed above which includes:

  1. Robust assessment to support treatment planning (i.e., person directed planning).
  2. Coordination of services including the identification of service gaps and referral needs.
  3. Ongoing monitoring of services to ensure they are occurring, are helping the individual progress towards their identified goals, and that the individual is satisfied with their services.
  4. Crisis management thru the coordination of services to support individuals with IDD remain safely and happily in the community.

The Candidate we are looking for is:

  • Excited to work with the IDD population. If you don't already have experience with this population, then you must be motivated to learn.

  • You have the ability to respond to deadlines in a timely manner.
  • Most importantly, you are looking for a career that makes a difference in peoples' lives.

Service Coordinator Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.

Service Coordinator License:

Required:

  • Current and valid driver's license and a clean driving record.
  • Center certification to perform service coordination. (Training will be provided during the onboarding process.)

Service Coordinator Experience:

Preferred:

  • Bilingual in Spanish.

Hours: Full-time; Monday-Friday, 8:00am-5:00pm, with some flexibility.

Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
