Education: Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field is required. Master's degree in social work, counseling, or related field is preferred.
Description: This Rehabilitation Specialist position is with Housing and Homeless Services and is located at the Community First! Village. Rehabilitation Specialists provide psychosocial rehabilitative skills training, intensive case management, and service coordination to individuals with complex needs, including psychiatric and/or co-occurring substance use disorders and chronic medical conditions, utilizing a person-centered, trauma-informed, recovery focused approach. Rehabilitation Specialists operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams and cultivate a clinical skill-set by forging therapeutic relationships with racially and culturally diverse community members. Frequent collaboration with internal and external providers is necessary to support individuals in maintaining housing, achieving independent functioning and building a sense of community.
Rehabilitation Specialists participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure personalized services and help define and facilitate the individual's desired life outcomes. Rehabilitation Specialists provide therapeutic interventions to assist diverse individuals in developing the skills necessary to manage symptoms, obtain/maintain employment or housing, and/or to obtain services such as medical care, nutritional assistance, financial assistance, transportation, legal assistance and resources fulfilling any basic need. Rehabilitation Specialists may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities.
This position will carry a high intensity caseload and also serves community residents on an outreach and on-call basis, as needed. Rehabilitation Specialists work under general supervision with moderate latitude for the use of initiative and independent judgment. This position requires the transportation of consumers in a personal and/or agency vehicle. Minimum two years direct client care experience in mental health required. Education may be substituted for experience.
License(s): Current and valid Texas driver's license and a clean driving record required.
Hours: Monday-Friday, 8:00am-5:00pm with flexibility to meet the needs of clients.
Starting Qualified Mental
Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but
internal applicants are subject to Integral Care's QMHP career ladder.
