Job Summary: The Integrated Clinic Case Manager will provide services in both a clinic and community setting as part of an integrated behavioral health clinic located in East Austin. Case managers operate from a client-centered approach and work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. This team works with clients who are currently experiencing, or have a history of, homelessness. The case manager has an understanding of the connection between biological, psychological, and social health and provide services utilizing a recovery based and trauma-informed perspective.  Case Managers operate in a dynamic environment, playing a key role in the multidisciplinary team within the Integrated Behavioral Health Clinic where the focus is on providing comprehensive care to adults diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and medical conditions. They develop extensive knowledge of community resources, collaborate with interdisciplinary teams and cultivate a clinical skill set by forging therapeutic relationships with diverse community members. 

Case Managers participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric, medical, and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. Case managers assess client education needs and gaps in health literacy, providing education and coordinating with other team members to address gaps. Case Managers work with individuals in developing skills necessary to manage symptoms, obtain/maintain employment or housing, self-manage medical conditions, or to obtain services such as specialty care, nutritional assistance, financial assistance, transportation, legal assistance and other basic needs resources. Case Managers may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities. Although staff members in this position are not typically LPHA's, staff members in this position are working under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment.

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. 


  • Graduated from an accredited college or university with a Masters degree with a major specialization in Human/Behavioral/Social Services or related field. 



  • Current and valid driver's license and a clean driving record.

Hours: Full-time; Monday - Friday

Salary$21.96 - $22.99 an hour based on education. "Internal applicants are subject to Integral Care's QMHP career ladder".
