Job Summary: The Case Manager is a Qualified Mental Health Professional (QMHP) who provides psychosocial rehabilitative and skills training services to adults housed at Foundation Communities Single Adult Studio apartments. The Clinical Case Manager is part of a multidisciplinary team who provides care along the continuum of services for adults with serious mental illness who may also have co-occurring substance use disorders, chronic health conditions, and/or IDD. The Case Manager works in partnership with individuals to definite and facilitate desired life incomes utilizing a recovery based and trauma-informed perspective.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

• Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field required. Masters Degree preferred.

License:

• Current and valid driver's license and a clean driving record.

Hours: Full-Time.

Workdays: Monday - Friday.

Workhours: 9:00AM - 5:00PM.

Salary: $24.18 - $25.39 an hour based on education.
