Job Summary: This position is with Housing and Homeless Services and is located at Community First! Village.  Rehabilitation Specialists provide recovery focused psycho-social rehabilitative skills training, intensive case management, and service coordination for individuals with high vulnerability and complex needs, including psychiatric and/or co-occurring substance use disorders, and chronic medical conditions.  This position operates in a dynamic environment and works in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes, utilizing a person-centered, trauma-informed, recovery focused approach. 

Rehabilitation Specialists provide therapeutic interventions to assist individuals in developing the skills necessary to obtain/maintain housing and employment, and more effectively manage their mental/behavioral health needs.  Rehabilitation Specialists also participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorders at regular intervals to ensure optimal and personalized services.  Frequent service coordination with external and internal providers is necessary to support individuals in achieving independent functioning, including accessing medical care, substance use treatment, nutritional assistance, financial assistance, transportation, legal assistance, or other services, as required to meet their goals and needs.  This position carries a high intensity caseload in addition to serving community residents on an outreach and on-call basis, when needed.  Rehabilitation Specialists may also facilitate psychosocial rehabilitative skills training groups on a variety of topics to help residents develop more effective coping strategies and achieve their recovery goals.

Rehabilitation Specialists work under general supervision with moderate latitude for the use of initiative and independent judgment.  This position requires the transportation of consumers in a personal and/or agency vehicle.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. 

Preferred:

  • Master's degree

License: 

Required:

  • Current and valid Texas driver's license and a clean driving record. 
  • Center certification (QHMP-CS) to perform case management 

Experience:

Required:

  • Minimum two years direct client care experience in mental health (Education may be substituted for experience).

Preferred:

  • Bilingual candidates 

Hours: Monday-Friday, 8:00am-5:00pm with flexibility to meet the needs of clients. 

Salary:  Starting Qualified Mental Health Professional (QMHP) salary is at $38,479 - $40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
