Permanent Supported Housing HCC - LPHA

Job Summary: This position is within Housing and Homeless services with the Permanent Supported Housing Healthcare Collaborative (PSH HCC). This program is a collaboration between Integral Care, CommUnity Care, Central Health, Dell Medical School and ECHO, designed to provide integrated, mobile healthcare to residents in designated Permanent Supported Housing single site projects. This program works to fill service gaps for adult residents existing chronic homelessness with disabling condition(s). Individuals served present with high vulnerability and complex needs. Preferred for this position will be a working knowledge of best practices for Permanent Supported Housing. LPHA may maintain a counseling caseload, and specialize in hoarding mitigation and eviction prevention planning.

The Licensed Professional of the Healing Arts (LPHA) provides assessment, triage, and treatment to individuals
diagnosed with mental illnesses who may also have co-occurring substance use disorders and/or chronic health
conditions using a recovery based and trauma-informed perspective.
LPHAs work in partnership with racially and culturally diverse individuals to define and facilitate their desired life
outcomes. LPHAs conduct diagnostic evaluations and provide assessment of strengths, preferences, functional
impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and
personalized services. LPHAs deliver therapeutic evidence-based and best practice interventions including, but not
limited to: skills training through psycho-education and patient education strategies, individual, family and/or group
services, and delivering crisis services with linkages to crisis resources as needed. On-call responsibilities may be required dependent upon unit protocols.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

  • Graduated from an accredited college or university with a Master's degree with a major specialization in Human/Behavioral/Social Services or related field.

License:

Required:

  • Current and valid driver's license and a clean driving record.

Experience:

Preferred:

  • Bilingual.

Hours: Full-Time.

Workdays: Monday-Friday.

Workhours: 8:00am-5:00pm (With Flexibility).

Salary: $64,798.24 - $82,610.32 annually depending on years of licensure.
