Job Summary: This position is within Housing and Homeless services with the Permanent Supported Housing Healthcare Collaborative (PSH HCC). This program is a collaboration between Integral Care, CommUnity Care, Central Health, and ECHO, designed to provide integrated, mobile healthcare to residents in designated Permanent Supported Housing single site projects. This program works to fill service gaps for adult residents exiting chronic homelessness with disabling condition(s). Individuals served present with high vulnerability and complex needs. Preferred for this position will be a working knowledge of best practices for Permanent Supported Housing. Interventions and services are typically provided in the community and require the use of a personal automobile and cell phone.
The primary responsibility of the Peer Specialist (PS) is to assist clients affected by mental and physical health and/or substance use concerns achieve their desired life outcomes. The PS provides all services in a person-centered, trauma-informed care, and culturally and linguistically appropriate manner. The PS acts as a model of recovery by sharing their lived experience to bring hope and engages clients by providing education, advocacy, and/or outreach. The PS is advanced in their recovery and is willing and able to share their recovery story in a positive, motivational manner. For some units, the PS position is reserved for individuals who have at least one cumulative year of receiving mental health services.Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.
