Job Summary: Integral Care's Oak Springs Clinic, in partnership with the Downtown Austin Alliance (DAA), Downtown Austin Community Court (DACC), and the Homeless Outreach Street Team (HOST), are leading a team specializing in providing community-based integrated health services to individuals experiencing homelessness in the downtown Austin corridor. We know individuals experiencing homelessness benefit from services in their respective environments; outside of the walls of a traditional clinic setting. By partnering with established outreach and engagement teams who have vested relationships with individuals prior to referral, we are confident in our aim to improve health outcomes for this population. Prospective members of this team have the opportunity to help fill a long-standing gap in service provision for our community: helping individuals experiencing homelessness sustain effective and meaningful connection to their treatment team en route to improved health and well-being. Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Hours: Full-Time; Workdays; Monday -Friday, Workhours; 8:00am-5:00pm
