Service Coordinator

Education: Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field is required.  Master's degree is preferred.

Description: The Service Coordinator performs moderately complex local authority/service coordination work assisting clients in accessing/linkage to medical, social, educational, financial, and other appropriate services/supports that may help individuals achieve a quality of life and community participation acceptable to the individual/legally authorized representative (LAR).  Works under general supervision with moderate latitude for the use of initiative and independent judgment.

The PASRR Service Coordinator (SC) completes the Evaluation as required by TMHP/HHSC and presents the Community Living Options to individuals who reside in Nursing Facilities and complete all related paperwork.  PASRR SC also completes enrollment activities for Medicaid waiver programs along with all related paperwork.  The PASRR SC also provides continuity of care services for individuals who have moved from the Nursing Facility to the community (ICF, HCS, etc.)

License(s): Center certification to perform service coordination is required.  Current and valid Texas driver's license and a clean driving record required. 

Hours: Monday-Friday, 8:00am-5:00pm.

Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
