Description: The Texas Home Living Program (TxHmL) provides essential services and supports so that people with IDD can continue to live with their families or in their own homes.  The General Revenue (GR) and Community First Choice (CFC) programs also provide services to individual who reside in their own home or family home, though the services are more limited. Services are designed to meet the person's needs so they may remain in the community and have opportunities to participate in their community to the maximum extent possible.

As a Service Coordinator you will monitor services and supports for individuals diagnosed with intellectual and/or developmental disabilities (IDD). In addition to IDD, many individuals you will serve will have mental health and/or medical diagnoses. Individuals served range in age from very young children to the geriatric population.  You will have a designated caseload of individuals for whom you will work with to help improve their quality of life.  You will provide these services in the community, including their place of residence, schools, day programming, etc.  The service coordinator role is a dynamic position that entails working with the individuals and their support systems, facilitating meetings, crisis prevention and advocacy.  As their service coordinator, you will not only monitor their TxHmL/GR/CFC Services, but also help support individuals in leading meaningful and fulfilling lives.

Responsibilities and Duties

  • Monitor progress, service delivery and satisfaction of services
  • Facilitate meetings amongst the service planning team
  • Complete and monitor individualized care plans
  • Identify and leverage clients own strengths to help them achieve their goals
  • Identify appropriate community resources and provide linkages to the individual and/or their family
  • Maintain documentation of all client contacts within required timelines and in compliance with program requirements
  • Maintain a caseload of TxHmL, GR, and/or CFC clients
  • Provide support to individuals in accessing appropriate services through communication within Integral Care, community partners, service providers and other relevant agencies.
  • Travel locally, daily, for client visits
  • Participate in inter-agency collaborations to problem solve client needs and barriers to progress
  • Utilize electronic health records, Microsoft Word/Outlook and other applications.

  • Bilingual preferred

Education: Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field is required

License(s): Center certification to perform service coordination is required. (Training will be provided during the onboarding process to obtain this certification). Current and valid Texas driver's license and a clean driving record required.  

Hours: Monday -Friday, 8am -5pm, with some flexibility required. 

Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder. Upon successfully passing our agency bilingual exam, an added annual bilingual pay up to $3,730 would apply.
