Description: The Intake Program determines the eligibility of individuals for the IDD Program.  Once the diagnosis of intellectual and/or developmental disabilities is determined by the psychologist, the individuals are assigned to the appropriate department to begin receiving services in the community. 

As an Intake Service Coordinator, you will meet with individuals in their homes who are interested in learning if they are eligible for IDD services.  At their homes, you will make scans of the documents which are needed to create their Integral Care chart.  You will explain the Intake process of the home appointment, the testing appointment at the office with the psychologist which is followed by meeting with another Intake Service Coordinator who explains the programs for which they are eligible.  Bilingual in Spanish preferred.

Responsibilities and Duties

  • Monitor the Intake Schedule for date/times and locations of intake appointments.
  • Meet with individuals in their homes. 
  • Obtain scans of required documents.
  • Maintain documentation of contact with individuals within required timelines and in compliance with program requirements.
  • Complete required forms and submit within the required timelines and in compliance with program requirements. 
  • Maintain the Intake Spreadsheet to ensure all Intake requirements are met.
  • Schedule testing appointments with the psychologist. 
  • Meet with individuals after testing appointments to explain their eligibility for services.
  • Assign eligible individuals to the appropriate supervisor. 
  • Participate in inter-agency collaborations to problem solve client needs and barriers to crisis prevention
  • Travel locally, daily, to meet with clients. 
  • Utilize electronic health records, Microsoft Word/Outlook and other applications.

Education: Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field is requiredMaster's degree preferred.

License(s): Center certification to perform service coordination is required. (Training will be provided during the onboarding process to obtain this certification). Current and valid Texas driver's license and a clean driving record required. 

Hours: Monday-Friday, 8:00am-5:00pm with some with some flexibility required.

Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
