ACT Care Coordinator (QMHP)

Job Summary: The primary purpose of position is to provide recovery focused intensive community based services for people served by the Assisted outpatient treatment (AOT) program. AOT is the practice of delivering outpatient treatment under court order to adults with severe mental illness who meet specific criteria, such as a prior history of repeated hospitalizations or arrest. It is a tool for assisting those individuals most at risk for the negative consequences of not receiving treatment.

The position provides coordination of community resources to assist consumers in accessing medical, social, educational, financial and other appropriate services that help the individual achieve an improved quality of life, while decreasing the need for inpatient hospitalization. This role works collaboratively with an interdisciplinary team to provide individual psychiatric rehabilitative skills training and crisis assessment/management in the consumer's home or in the community environment. The position requires strong engagement and harm reduction intervention skills and effective collaboration with various community partners from the hospital and criminal justice field. At this advanced level, consumer caseloads are complex with increased need for risk assessment and crisis intervention. Work is performed under limited supervision of the unit manager with extensive latitude for the use of initiative and independent judgement.

On-call responsibilities may be required dependent upon unit protocols. This position may have additional duties associated with providing bilingual communication services and requires the completion of a language competency test.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. 

Preferred:

  • Graduated from an accredited college or university with a Master's degree with a major specialization in Human/Behavioral/Social Services or related field. 

License: 

Required:

  • Current and valid driver's license and a clean driving record.
  • Center certification as a Qualified Mental Health Profession (QHMP). (This certification will be provided by Integral Care if educational requirements are met.)

Hours: Full-time; Workdays; Monday-Friday, Workhours; 8:00am-5:00pm 

Salary: $22.35 - $23.47 an hour based on education.
