Job Summary: The Homeless Outreach Street Team (HOST) is a partnership between Integral Care, the Downtown Austin Community Court, the ATCEMS Community Health Paramedics, and the Austin Police Department engaging in outreach services with people experiencing homelessness in downtown Austin. HOST provides services in the community wherever individuals experiencing homelessness may be found, including city streets, parks, churches, and at partner homeless service agencies.
QMHP responsibilities with HOST include locating and engaging people experiencing homelessness; helping meet people's needs to reduce emergency department, psychiatric hospital, and criminal justice involvement; completing coordinated housing assessments; assisting with applications for housing, benefits, or vital documents; referrals to mental health or substance abuse services; assisting with scheduling appointments; providing transportation to appointments in HOST's vehicles as indicated; connecting clients with shelter; and assisting with basic needs such as food, water, and clothing. Ideal candidates will be adaptable, resourceful, persistent, and work well as a member of an interdisciplinary team.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Salary: $23.25 - $24.41 an hour based on education.
