Job Summary: The Community Health Worker is a Qualified Mental Health Professional (QMHP) plays a strategic role in engaging individuals to define and facilitate desired life outcomes utilizing a recovery based and trauma-informed approach. The Community Health Worker assists adults with serious mental illness who may also have co-occurring substance use disorders, chronic health conditions and/or IDD with connection to ongoing health services, promotes communication and understanding among providers, improves adherence to health recommendations, and decreases the use of emergency services. Interventions and services are designed to meet the person wherever is most convenient to them, occurring in the integrated adult behavioral health clinic as well as a variety of community settings.
This position may have additional duties associated with providing bilingual communication services and requires the completion of a language competency test.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Hours: Full-time; Monday - Friday, Hours; 8:00AM - 5:00PM.
Salary: $22.35 - $23.47 an hour based on education. "Internal applicants are subject to Integral Care's QMHP career ladder".
