Service Coordinator (QMHP)

Job Summary: The Preadmission Screening & Resident Review (PASRR) program is a federally mandated program that requires all states to prescreen all people seeking admission to a Medicaid-certified nursing facility, regardless of payer source or age. The program seeks to ensure appropriate services and placement for these individuals. 

This position focuses primarily on individuals with a confirmed mental health diagnosis who reside in Medicaid-certified nursing facilities throughout Travis County.  Service coordination provided by this program can include: 

  • Regular clinical assessment and evaluation to support person-centered treatment planning, helping the individual better define and facilitate their desired life outcomes. 
  • Coordination of services including the identification of service gaps and referral needs. 
  • Ongoing monitoring of services to ensure they are occurring, are helping the individual progress towards their identified goals, and that the individual is satisfied with their services. 

Service Coordinators work under general supervision of the unit manager with moderate latitude for the use of initiative and independent judgment. 

The ideal Candidate is someone who:

  • Is excited to work with a largely older adult population - prior experience in this area is not required, just a willingness to learn  
  • Enjoys forging therapeutic relationships with people from many different cultural backgrounds 
  • Is self-motivated, independent, flexible, and appreciates variety in their work day 
  • Communicates effectively and professionally all kinds of people. 
  • Is organized, reliable, and skilled in time management

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. 
Preferred:
  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. 

Licenses:

  • Current and valid Driver's License and a clean driving record are required.
  • Center certification as a Qualified Mental Health Professional (QMHP) required.

Hours: Full-Time; Workday; Monday - Friday, Workhours; 8:00am - 5:00pm

Salary: $22.35 - $23.47 an hour based on education. .
