Job Summary: The Care Coordinator/Specialist provides psychosocial rehabilitative skills training to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions, utilizing a recovery based and trauma-informed perspective. Care Coordinators/Specialists operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill-set by forging therapeutic relationships with diverse community members. Interventions and services are typically provided in the community and require the use of a personal automobile and cell phone.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Hours: Full-Time.
Workdays: Monday - Friday (occasional on-call nights).
Workhours: 8:00 AM - 5:00 PM (occasional on-call nights).
Salary: $24.18- $25.39 an hour based on education, plus an additional 10% crisis stipend.
