Job Summary:  The Pre Admission Screening and Resident Review (PASRR) Program serves people diagnosed with intellectual and/or developmental disabilities who are potentially admitting to or reside in a nursing facility setting. PASRR Coordinators assist individuals with accessing specialized services and with accessing community programs to be able to live in a community setting with supports.

As a PASRR Coordinator, you will monitor services and supports for individuals diagnosed with intellectual and/or developmental disabilities (IDD). In addition to IDD, many individuals you will serve will have mental health and/or medical diagnoses.  You will have a designated caseload of individuals for whom you will work with to help improve their quality of life. The PASRR Coordinator facilitates quarterly service planning team meetings for individuals on their caseloads, which includes the individual, active family members, and nursing facility professional staff. The role is a dynamic position that entails working with the individuals and their support systems, creating person centered service plans, assisting in transitioning to community settings, and advocacy.  As their PASRR Coordinator, you will not only monitor their PASRR Services, but also help support individuals in leading meaningful and fulfilling lives.

Responsibilities and Duties

  • Monitor progress, service delivery and satisfaction of services
  • Facilitate meetings amongst the service planning team
  • Complete assessments and service plans to support the individuals preferences and needs
  • Identify and leverage clients own strengths to help them achieve their goals
  • Maintain documentation of all client contacts within required timelines and in compliance with program requirements
  • Develop working relationships with nursing facilities and community providers
  • Assist individuals in finding placements
  • Maintain a caseload of approximately 28 - 38 clients
  • Provide support to individuals in accessing appropriate services through communication within Integral Care, community partners, service providers and other relevant agencies.
  • Travel locally, daily, for client visits
  • Participate in inter-agency collaborations to problem solve client needs and barriers to progress
  • Utilize electronic health records, Microsoft Word/Outlook and other applications.

Education: Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field is required.  Master's degree is preferred.

Licenses:Center certification to perform service coordination is required. (Training will be provided during the onboarding process to obtain this certification.) Current and valid Texas driver's license and a clean driving record required. 

Hours:Monday -Friday, 8am-5pm with some with some flexibility required

Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
