Job SummaryCommunity-Based Rehab Specialists are clinicians who provide timely face to face follow-up with individuals discharging from inpatient psychiatric hospitals in order to ensure a safe transition back into the community.

Community-Based Rehab Specialists provide psychosocial rehabilitative skills training to adults diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective.  Community-Based Rehab Specialists operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams and cultivate a clinical skill-set by forging therapeutic relationships with diverse community members.  Interventions and services are typically provided in local psychiatric facilities and the community and require the use of a personal automobile and cell phone.

Community-Based Rehab Specialists work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes.  Community-Based Rehab Specialists participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services.  Community-Based Rehab Specialists deliver therapeutic interventions fostering skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as medical care, nutritional assistance, financial assistance, transportation, legal assistance and resources fulfilling any basic need.  Community-Based Rehab Specialists function as agency advocates and work closely with local psychiatric hospital staff while linking individuals to their service providers through Integral Care as well as assisting with discharge planning needs.  Community-Based Rehab Specialists often function in external hospital settings and are expected to maintain professional relationships while representing Integral Care's mission. In addition to maintaining contact with Integral Care consumers admitted to local psychiatric facilities, Community-Based Rehab Specialists will carry a small, flexible caseload of consumers that may need additional clinical support when discharging from the hospital or for the purpose of crisis prevention. Frequent service coordination with external and internal providers is necessary to support individuals in achieving independent functioning.

Community-Based Rehab Specialists may also facilitate psychosocial rehabilitation skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities. Although staff members in this position are not typically LPHA's, staff members in this position are working under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment. 

On-call responsibilities may be required dependent upon unit protocols. 

Works under general supervision with moderate latitude for the use of initiative and independent judgment. 



  • Graduation from an accredited four-year college or university with a major in a social, behavioral, or human service field.


  • Current and valid Driver's License and a clean driving record.
  • Center certification as a Qualified Mental Health Profession (QHMP) to perform psychosocial rehabilitative skills training and case management. (This certification will be provided by Integral Care if educational requirements are met.)

Hours: Full-Time; Monday - Friday, 8:00 am-5:00 pm.

Salary: $21.96 - $22.99 an hour based on education. "Internal applicants are subject to Integral Care's QMHP career ladder".
