Job Summary:

Village and provides mental and behavioral health services, substance use support, and case management to its residents. 

Rehabilitation Specialists provide psychosocial rehabilitative skills training, emotional support, intensive case management, and service coordination to vulnerable individuals with complex needs, including psychiatric and/or co-occurring substance use disorders, and chronic medical conditions.  Rehabilitation Specialists utilize a person-centered, trauma-informed, recovery focused approach to forge therapeutic relationships with racially and culturally diverse individuals, helping them define and facilitate desired life outcomes, cultivate independent living skills, maintain housing, and more effectively manage their mental and behavioral health needs.   

Rehabilitation Specialists participate in diagnostic evaluations at regular intervals and provide assessment of individual strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring substance use disorders to ensure optimal and personalized services.  Frequent service coordination with external and internal providers is necessary to support individuals in achieving independence and may include access to medical care, substance use treatment, nutrition, financial assistance, transportation, legal assistance, or other services as required to meet their goals and needs.  Rehabilitation Specialists may also facilitate psychosocial rehabilitative skills training groups to help individuals develop more effective coping strategies and achieve their recovery goals.  This position carries a high intensity caseload in addition to serving community residents on an outreach and on-call basis when needed.   

Rehabilitation Specialists work under general supervision with moderate latitude for the use of initiative and independent judgment.  Although staff members in this position are not typically LPHA's, staff members in this position work under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses, and treatment.  This position requires the transportation of consumers in a personal and/or agency vehicle. Master's degree in social work, counseling, or related field and/or two years direct client care experience in mental health strongly preferred.  



  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field


  • Graduated from an accredited college or university with a Master's degree with a major specialization in Human/Behavioral/Social Services or related field 



  • Current and valid driver's license and a clean driving record
  • Center certification to perform service coordination


  • LPC-Associate or LMSW License



  • Two years community mental health experience

Hours: Full-time; 8:00 AM - 5:00 PM with flexibility for on-call rotation.

SalaryStarting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder
