WRAP Facilitator - QMHP

Job Summary: This position involves providing intensive case management and facilitating Wraparound services in accordance with the Wraparound Model developed by the National Wraparound Initiative Center. 

Key responsibilities include:

  • Assisting youth and their families in identifying and accessing appropriate mental health, medical, social, educational, financial, and other services and supports. 
  • Assessing youth needs and strengths to authorize services and manage ongoing service authorizations.
  • Coordinating service delivery with a multi-disciplinary team to provide an array of services to children and youth enrolled in the Youth Empowerment Services Waiver program.
  • Overseeing the development of the Wraparound Plan and Safety Plan, including crisis prevention and management as needed.
  • Monitoring service delivery and ensuring the development and coordination of the wraparound team for each youth on the caseload.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. Master's degree preferred.

License:

Required:

  • Current and valid driver's license and a clean driving record.

Experience:

Preferred:

  • Bilingual in Spanish.

Hours: Full-Time.

Workdays: Monday - Friday.

Workhours: 8:00AM - 8:00PM.

Salary: $24.18 - $25.39 an hour based on education.

Position may be eligible for an additional shift differential pay.

  • Second Shift: $3.25/hour.
  • Third Shift: $7.25/hour.
  • Weekend Shift: $8.00/hour.
