Job Summary: The PASRR program provides mental health screening, assessment, and treatment to individuals residing in Medicaid certified skilled nursing facilities. Individuals served are largely older adults, however there are no age restrictions or requirements for PASRR services.

PASRR services utilize a person-centered, trauma-informed, recovery-focused approach to forge therapeutic relationships with racially and culturally diverse individuals to help them:

  • Define and facilitate desired life outcomes.
  • Cultivate independent living skills.
  • Process complex grief and adjustment issues.
  • Build more effective coping skills to better manage their mental and behavioral health needs.
  • Locate and secure independent housing, financial assistance, transportation, or other resources necessary to achieve their goals
The primary role of the PASRR Team Lead is to provide additional support for the PASRR team and assist with problem solving, organization, resource acquisition, and client advocacy. The PASRR Team Lead also acts as a liaison with community partners, nursing facility staff, and the State Health and Human Services Department, when necessary.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. Master's degree preferred. 

License:

Required:

  • Current and valid driver's license and a clean driving record.

Experience: 

Preferred:

  • Bilingual in Spanish.

Required:

  • 2 years of experience working directly with individuals with serious mental illness.

    Hours: Full-Time.

    Workdays: Monday - Friday.

    Workhours: 8:00AM - 5:00PM.

    Salary: $30.86, an hour, based on education.
