Rehabilitation Specialist

Job Summary: This position operates in a dynamic environment and works in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes and utilizes the evidence-based practices of Housing First, Harm Reduction, and Trauma Informed Care. Rehabilitation Specialists also participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorders at regular intervals to ensure optimal and personalized services.  Frequent service coordination with external and internal providers is necessary to support individuals in achieving independent functioning. This position will carry a high intensity caseload and serve residents on an outreach and on-call basis, as needed. 

The Rehabilitation Specialist will facilitate community engagement and group activities. Rehabilitation Specialists will build on strength by fostering an attitude of hope, respect, and compassion. 

Rehabilitation Specialists work under general supervision with moderate latitude for the use of initiative and independent judgment.  This position requires the transportation of consumers in a personal and/or agency vehicle. 

Rehabilitation Specialist Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. 

Rehabilitation Specialist License: 

Required:

  • Current and valid driver's license and a clean driving record. 

Hours: Full-time; Monday-Friday, 8:00 AM - 5:00 PM with flexibility. 

Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
