Case Manager Job Summary:The Case Manager will work collaboratively with both the school and the clients/families that they are serving in order to reduce truancy and improve school engagement. This role is responsible for effectively engaging families and students identified as needing support in school attendance. This role will work with students and their families to create a plan of care based on strengths and needs, closely monitor the implementation of the plan and maintain a strong partnership with the school. This position will work both in the community and school setting. Integral Care uses an electronic medical record and all documentation will be monitored closely. Applicants must have strong computer skills.

Hours: Full-time; Monday-Friday, 8:00am -5:00pm with some flexibility for evening hours.
Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479 - $40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
