Job Summary: The primary purpose of position is to provide recovery focused intensive community based services for people served by the Assisted outpatient treatment (AOT) program. AOT is the practice of delivering outpatient treatment under court order to adults with severe mental illness who meet specific criteria, such as a prior history of repeated hospitalizations or arrest. It is a tool for assisting those individuals most at risk for the negative consequences of not receiving treatment. This service is done by working collaboratively with an interdisciplinary team, using the ACT model, to assist consumers to foster a connection to the community and build support networks that promote wellness, while decreasing the need for inpatient hospitalization and/or incarceration. The position will require strong engagement and harm reduction intervention skills. This position will require effective collaboration with various community partners from the hospital and criminal justice field. The specialized responsibility of this position if to coordinate and monitor the holistic needs of clients served.
The Care Coordinator/Specialist provides psychosocial rehabilitative skills training to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective. Care Coordinators/Specialists operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams and cultivate a clinical skill-set by forging therapeutic relationships with diverse community members. Interventions and services are typically provided in the community and require the use of a personal automobile and cell phone.
Care Coordinators/Specialists work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. Care Coordinators/Specialists participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. Frequent service coordination with external and internal providers is necessary to support individuals in achieving independent functioning. Care Coordinators/Specialists may provide all or some of the following: deliver therapeutic interventions fostering skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need. Care Coordinators/Specialists may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities. Although staff members in this position are not typically LPHA's, staff members in this position are working under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment.
Hours: Full-time; Monday - Friday, 8:00 AM - 5:00 PM.
Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
