Job Number: 21-638-11481
Integral Care's Oak Springs Clinic, in partnership with Downtown Austin Alliance (DAA), Downtown Austin Community Court (DACC), and the Homeless Outreach Street Team (HOST), will be leading a new team specializing in providing community-based integrated health services to individuals experiencing homelessness in the downtown Austin corridor. We know individuals experiencing homelessness benefit from services in their respective environments; outside of the walls of a traditional clinic setting. By partnering with established outreach and engagement teams who have vested relationships with individuals prior to referral, we are confident in our aim to improve health outcomes for this population. Prospective members of this team have the opportunity to be a part of the mission from the beginning and help fill a long-standing gap in service provision for our community: helping individuals experiencing homelessness sustain effective and meaningful connection to their treatment team en route to improved health and well-being. If your vision of your next career move involves working closely with a group of socially-conscious, collaborative, and creative teammates committed to social justice and dedicated to partnering with some of our community's most vulnerable individuals, we look forward to hearing from you.
The QMHP Team Lead is responsible for leading the team's service delivery and ensuring the provision of recovery- focused, person-centered care with a trauma-informed perspective. The QMHP Supervisor operates in a dynamic environment, develops extensive knowledge of community resources, collaborates with interdisciplinary teams and cultivates a clinical skill-set by forging therapeutic relationships with diverse community members. The QMHP Supervisor participates in diagnostic evaluations and provides assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) to ensure optimal and personalized services. The QMHP supervisor identifies and implements strategies for ongoing program development and achievement of unit performance measures and engages staff to ensure the highest quality of care and effective use of provider staff time and resources.
