Service Coordinator

Job Summary: Service Coordinators perform moderately complex local authority/service coordination work assisting clients in accessing/linkage to medical, social, educational, financial, and other appropriate services/supports that may help individuals achieve a quality of life and community participation acceptable to the individual/legally authorized representative (LAR). PASRR also provides continuity of care services for individuals who have moved from the Nursing Facility to the community (ICF, HCS, etc).

Works under general supervision with moderate latitude for the use of initiative and independent judgment.

PASRR MI Division completes the PL1 and PASRR Evaluation as required by TMHP/HHSC designed to validate the suspected diagnosis of Mental Illness (MI). The PASRR Evaluation (PE) is designed to ensure the individual is appropriate for placement in a nursing facility and receiving the specialized services needed to improve and maintain the individuals' level of functioning. PASRR adheres to strict timelines per the TAC, Title 40 Part 1 Chapter 17 and all related paperwork must be completed using the LTC (Long-Term Care) Online Portal and Agency EHR System. All evaluations are community based and completed in Skilled Nursing Facilities. 

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.

Preferred:

  • Graduated from an accredited college or university with a Master's degree with a major specialization in Human/Behavioral/Social Services or related field

License:

Required:

  • Current and valid driver's license and a clean driving record.

Experience:

Preferred:

  • Bilingual in Spanish

Hours: Full-time; Monday - Friday; 8:00 AM - 5:00 PM

SalaryStarting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
