Job Summary: This position is within Integral Care's Housing and Homeless Services Division. This is an exciting and new opportunity to provide support and partner with individuals as they transition from homelessness into permanent, stable housing. This position and team provide site-based supportive housing services where they are needed most - where a person lives.
Rehabilitation Specialists work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. Rehabilitation Specialists participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. Frequent service coordination with external and internal providers is necessary to support individuals in achieving independent functioning.
Rehabilitation Specialists may provide all or some of the following: deliver therapeutic interventions fostering skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need. Rehabilitation Specialists may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities. Although staff members in this position are not typically LPHA's, staff members in this position are working under the supervision of an LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses, and treatment.

Hours: Full-time; Tuesday, Wednesday, Thursday and Friday, 8:00am-6:00pm.
Salary: $22.35 - $23.47 an hour based on education.
