Job Summary: This position may be located in any service unit of Integral Care. The primary purpose of the position is to assess a client's mental impairment, develop a personalized plan to provide services sought by the client, coordinate appropriate services and provide necessary skills training to facilitate the client's success in achieving and maintaining the maximum possible level of independent functioning. Services may include developing skills necessary to obtain/maintain employment or housing, or to obtain services such as medical care, dental care, food, clothing, utilities, financial assistance, transportation, legal assistance, or any other basic human need and the coordination of these services to assist the client in obtaining these services. Client caseloads are relatively stable with limited need for crisis intervention. Work is performed under the general supervision of the unit manager with moderate latitude for the use of initiative and independent judgment. On call rotation is required for after-hour crisis services.
Hours: Full-time; Monday-Friday, 8:00am-5:00pm, with flexibility to meet the needs of the client.
Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
