Job Summary: The position is responsible for ensuring the provision of recovery-focused, person-centered care with a trauma-informed perspective to persons experiencing substance use and other co-occurring disorders. This position operates in a dynamic environment and performs case management, assessment, and/or service coordination work based on the program's guidelines. Position related substance use duties may include and are not limited to providing continuity of care services, linking consumer and families to all needed resources, navigating the substance use services system within Region 7, and crisis prevention. This position is responsible for coordinating, monitoring and updating consumer treatment plan goals and objectives within the agency electronic health record. This position will efficiently refer and coordinate to clinically appropriate, internal agency resources. A working knowledge of substance use disorders, co-occurring mental health disorders and infectious disease is preferred. Bilingual preferred. Interventions and services are typically provided in the community and require the use of a personal automobile.

Salary: $22.35 - $23.47 an hour based on education.
