Job Summary:This position is with the Integrated Value Based Care team under Accountable Care Systems. Case Managers provide services to racially and culturally diverse individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective. Case Managers operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate clinical skills. Interventions and services may be office or community based and may require the use of a personal automobile.

Case Managers participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. They work in partnership with individuals to define and facilitate their desired life outcomes as well as skills.

Case Managers work with individuals in developing skills necessary to manage symptoms, obtain/maintain services such as medical care, nutritional assistance, financial assistance, transportation, legal assistance, housing or other resources. Case Managers may also facilitate psychosocial rehabilitative skills training sessions and/or utilize other evidenced-based interventions or therapeutic modalities. Individuals served at this level are assessed for crisis services as needed.

Although staff members in this position are not typically licensed mental health professionals, staff members in this position work under the supervision of a licensed mental health professional who provides ongoing evaluation and oversight of assessments, diagnoses and treatment.

This position may have additional duties associated with providing bilingual communication services and may require the completion of a language competency test.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. 

License: 

Required:

  • Current and valid Texas driver's license and a clean driving record. 

Experience:

Preferred:

  • Bilingual in Spanish 

Hours: Full-Time, Monday-Friday.

Salary:  Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
