Outreach and Engagement Specialist Job Summary: Outreach and Engagement Specialists provide services to individuals experiencing homelessness who may be diagnosed with severe mental illness. They may also have co-occurring substance use disorders and/or chronic health conditions and treatment is provided utilizing a recovery based and trauma-informed perspective. Outreach and Engagement Specialist operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill set by forging therapeutic relationships with diverse community members. Interventions and services can be both office and community based and may require the use of a personal automobile and cell phone.
Outreach and Engagement Specialist work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. Outreach and Engagement Specialists participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. Outreach and Engagement Specialists work with individuals in developing skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as medical care, nutritional assistance, financial assistance, transportation, legal assistance and resources fulfilling any basic need. Individuals served at this level may be assessed for crisis services as needed. Although staff members in this position are not typically LPHA's, staff members in this position are working under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment.

Hours: Full-time; Monday-Friday, 8:00am-5:00pm.
Salary: $21.96 - $22.99 an hour based on education. "Internal applicants are subject to Integral Care's QMHP career ladder".
