Job Summary: This is a remote position with the Integrated Behavioral Health Home (IBHH) under Accountable Care. The Licensed Professional of the Healing Arts (LPHA) provides complex care coordination, assessment, triage, and treatment to racially and culturally diverse individuals diagnosed with mental illnesses who have a comorbid substance use disorder and/or chronic health condition. Treatment occurs under a recovery based and trauma-informed perspective. LPHAs operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and utilize clinical skills. Interventions and services may be office or community based, and require the use of a personal automobile.
LPHAs conduct diagnostic evaluations, and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. Using therapeutic interventions, they work in partnership with individuals to define and facilitate desired life outcomes.
LPHAs deliver therapeutic evidence-based practices that include, but not limited to, skills training, psychoeducation, individual, family, or group sessions, and crisis services. This position is a key role within the IBHH and requires frequent coordination with external and internal providers as necessary to support individuals in treatment.
LPHAs work with individuals in developing skills necessary to manage symptoms. Treatment includes ensuring individuals are engaged and able to access services such as medical care, financial assistance, transportation, specialty services, housing, or other resources.Education:

Salary: $62,306.00 - $79,433.00 annually, depending on licensed experience.
