Job Summary:This position is located in Child and Family Service Division of Integral Care providing services to youth identified as having Serious Emotional Disturbances (SED). The youth and families eligible for intensive case management are among the highest need youth within Travis County. This position requires the ability to multitask in a fast pace environment, work collaboratively with unit staff and community partners, and to successfully direct a team of inter-disciplinary and nontraditional service providers.
This position provides intensive case management and Wrap facilitation using the Wraparound Model as designed by the National Wraparound Initiative Center. Wraparound is an ecologically based process and approach to care planning that builds on the collective action of a committed group of family, friends, community, professional, and cross-system supports mobilizing resources and talents from a variety of sources resulting in the creation of a plan of care that is the best fit between the family vision and story, team mission, strengths, needs, and strategies.
Examples of work performed include: Assisting youth and parent(s) in identifying medical, social, educational, financial, and other services and supports as appropriate or needed. Assess youth's needs and strengths to authorize services, and continued management of authorizations. Coordinate service delivery with a multi-disciplinary team and other community providers. Coordinate the development of the Wrap Plan and Safety Plan, including crisis prevention and management when needed. Monitor service delivery and development of wrap team for each youth on case roster.
Hours: Full-Time; Monday - Friday, 8:00 am-8:00 pm; varied with flexibility.
Salary: Starting Qualified Mental Health Professional (QMHP) salary is $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
