Job Summary:This position is to perform intensive case management and Wrap facilitation using the Wraparound Model as designed by the National Wraparound Initiative Center. Examples of work performed include: Assisting youth and parent(s) in identifying medical, social, educational, financial, and other services and supports as appropriate or needed. Assess youth's needs and strengths to authorize services, and continued management of authorizations. Coordinate service delivery with a multi-disciplinary team and other community providers. Coordinate the development of the Wrap Plan and Safety Plan, including crisis prevention and management when needed. Monitor service delivery and development of wrap team for each youth on case roster.

Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
