Education: Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field is required. Master's degree preferred.
Description: This position may be located in any direct service division of Integral Care, the MHMR authority for Travis County. Its purpose is to perform intensive service coordination utilizing the Wraparound Model as designed by the National Wraparound Initiative Center. Examples of work performed include: Assist in accessing medical, social, educational, financial, and other appropriate services; Assess client's needs and authorizes services to meet the identified needs; Coordinate service delivery; Coordinate the development of the Person Centered Care Plan; Provide crisis prevention and management services when needed; Monitor service delivery. 2 years of paid case management experience is preferred.
License: Current and valid Texas driver's license and a clean driving record required. Center certification to perform service coordination is required.
Hours: Full-Time; Monday-Friday, 9:00am-5:00pm.
Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
