Service Coordinator (QMHP)

Job Summary: The Service Coordinator works alongside a team of advocates to help adults with Intellectual and Developmental Disabilities (IDD) seeking admission to a Medicaid-certified nursing facility, who may also have a co-occurring mental health diagnosis, to ensure access and continuity of care.

The Service Coordinators' day-to-day activities follow the federally mandated Preadmission Screening and Resident Review (PASRR) program guidelines. The PASRR program screens people seeking admission to ensure appropriate placement and access to required services. Service Coordinators routinely use state electronic systems such as CARE and TMHP and can facilitate transition duties when an individual requests to move out of the nursing facility. To the extent required, a Habilitation Coordinators' focus may shift to primarily support functions, which can be referred to as PASRR evaluator tasks.

Service Coordinators apply a person-centered approach and perform a set of four service coordination elements - assessment, service planning/coordination, monitoring, and crisis prevention/management. Efforts commonly include working within a multidisciplinary team consisting of internal or external community partners.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.

License:

Required:

  • Current and valid driver's license and a clean driving record.

Experience:

Required:

  • Have at least one year of experience working directly with individuals with ID or DD.

Hours: Full-Time.

Workdays: Monday - Friday.

Workhours: 8:00AM - 5:00PM.

Salary: $22.35 - 23.47 an hour based on education.
