Description: This position performs moderately complex local authority/service coordination work assisting clients in accessing/linkage to medical, social, educational, financial, and other appropriate services/supports that may help individuals achieve a quality of life and community participation acceptable to the individual/legally authorized representative (LAR).  Works under general supervision with moderate latitude for the use of initiative and independent judgment.

PASRR completes the Evaluation as required by TMHP/HHSC and presents the Community Living Options to individuals who reside in Nursing Facilities and complete all related paperwork.  PASRR also completes enrollment activities for Medicaid waiver programs along with all related paperwork.  PASRR also provides continuity of care services for individuals who have moved from the Nursing Facility to the community (ICF, HCS, etc.)

Responsibilities and Duties

  • Monitor progress, service delivery and satisfaction of services
  • Facilitate meetings amongst the service planning team
  • Complete and monitor individualized care plans
  • Identify and leverage clients own strengths to help them achieve their goals
  • Maintain documentation of all client contacts within required timelines and in compliance with program requirements
  • Develop working relationships with community HCS providers

  • Assist individuals in finding HCS placements.
  • Maintain a caseload of approximately 40 client
  • Provide support to individuals in accessing appropriate services through communication within Integral Care, community partners, service providers and other relevant agencies.
  • Travel locally, daily, for client visits
  • Participate in inter-agency collaborations to problem solve client needs and barriers to progress
  • Utilize electronic health records, Microsoft Word/Outlook and other applications.

Education: Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field is required

License Center certification to perform service coordination is required. (Training will be provided during the onboarding process to obtain this certification.) Current and valid Texas Driver's License and a clean driving record are required.

Hours: Monday -Friday, 8am-5pm with some with some flexibility required.

Salary: Starting Qualified Mental Health Professional (QMHP) salary is at $38,479-$40,739 based on education, but internal applicants are subject to Integral Care's QMHP career ladder.
